The Healthy Food Center has 5 locations – West Penn Hospital, Allegheny General Hospital, Jefferson Hospital, Forbes Hospital, and Saint Vincent Hospital. We are partnering with The Greater Pittsburgh Community Food Bank to provide patients with a variety of healthy foods. The model of the Healthy Food Center is to provide nutritious food and education to patients who are food insecure. Since AHN has implemented a new Social Determinants of Health screening across the network, we are able to identify patients based on the two of the questions that screen for food security. If a patient answers positive on either of the questions, they are given an order to come to the Healthy Food Center from their doctor. Here, they can receive nutritious food, which will be based on household size and health conditions, along with education from our Healthy Food Center staff comprised of Registered Dietitians and Dietetic Technicians. Patients are also offered to schedule follow up nutritional counseling appointments if they choose to do so. There will be community wrap around resources and services provided as well, such as completing WIC and SNAP applications. Note: * There is not specific intern start or end dates- flexible.
